The Medical and Liability Release Form  (yellow) as well as the Parent Waiver and Student Agreement Form (green) should be filled out and mailed to the Band Boosters at the Post Office box address referenced in the Beginning of the Year Information Packet - the document for everything you need to know about the Nease Band!
