Click HERE to fill out the student information form referenced in your 2009-2010 information packet. Every band member including guard and percussion MUST fill out this form online. The Medical and Liability Release Form (green) as well as the Parent Waiver and Student Agreement Form (orange) should be filled out and mailed to Sherry Zastera in the envelope provided. Additionally, you may also send the volunteer form (pink)to Sherry Zastera. Thank you!
